下垂体間腦性緑内障に就て

Pituitary-diencephalic Glaucoma

緒言

　眼圧が間脳と関係があることについては既に多くの研究がある。間脳の自律中枢を電気的に刺戟すると眼圧変動が起ることは永井^1），高木^2），金田^3），赤木^4），その他^5）の諸氏によつて認められている。又Kahler u．　Sallmann （1923）^6），Lowensteinu．Schonberg （1944）^7），Alajmo and Rubino （1948）^8），Elwyn （1950）^9），　Thiel （1952）^10），赤木^4）（昭和31）等の諸氏は何れも眼圧調節中枢が間脳にあることを論じている。間脳は植物神経の中枢としてすべての内臓諸臓器に対すると同様に，眼（眼圧）にも調節的支配を興えているというのである。斯くして緑内障とは一の全身病で特に眼部にその病変を示すものであるという意見も存在する。此の意味で緑内障というよりもglaucomatic diseaseと呼ぶ方が適切だとWeinstein^11）は云つている。緑内障の原因を全身的のneurovascular又はneurohumoralの因子に求めようとする考えは，眼局所の機械的因子に求めるmechanical schoolと共に緑内障原因論の二大主流をなしている。

　眼圧と内分泌特に下垂体との関係も古くから注目^12）され前葉ホルモン，後葉ホルモンの眼圧に対する作用が種々検討されている。

This is a report of simple glaucoma in a dwarf with high myopia.

The patient is a 16 years old female of 121.5. cm. high and with body weight of 27 Kgs.

The vision is : 0.D.=0.06 (0.3×-18. 0D) 0. S. =0.08 (0.3-17.0D)

The field of vision is normal.

The fundus is that of high myopia without glaucomatous change. There is a definitely increased diurnal variation of ocular tension : in the right eye in the range of 19.0 and 44.8 mmHg., and in the left eye in the range of 21. 7 and 56.0 mmHg.

The Suda's compression test, lability test, and water drinking test are positive while the dark-room test is negative.
