Summary

 Bronchoscopic alveolar lavage(BAL) is an efficient treatment for pulmonary alveolar proteinosis(PAP) which is characterized by an excessive accumulation of aggregated lipoproteins in the alveolar space. N-acetyl-L-cysteine(NAC) which is a well-characterized thiol-containing agent, efficiently reduces the disulfide bonds of sulfhydryl-containing proteins. Reportedly, the structure of surfactant proteins with PAP is characterized for the most part by thiol-dependent aggregates. A case of pulmonary alveolar proteinosis was treated by BAL using a NAC-containing solution, and clinical improvement was observed after this treatment. In this case, we measured the quantity of aggregates collected by BALF using saline with or without NAC. The results of our measurements demonstrated that the quantity of aggregates collected by BAL using NAC solution was higher than that using saline only. In addition, we found that NAC dissolves the aggregates efficiently in vitro. These observations implied that NAC solution might be valuable in the treatment of PAP by lung lavage.
