急性白血病の1例で見られた網膜の毛細血管瘤

Capillary Microaneurysms of the Retina in Acute Leukemia

緒言

　白血病患者において，その生前および剖検で，種々の眼症状と眼組織変化の見られることは，古くから多くの記載がある^1〜5）。

　病変は，組織学的には，白血病細胞の浸潤と出血が主であるが，臨床症状は症例ごとに多彩である。加えて近年では，化学療法や血小板輸血などの発達により，白血病の診断後，寛解・増悪を繰返しつつ，年余にわたつて生存しうる者が次第に増加しているので^6），眼科領域でも，そのような白血病の病像変遷と相挨つて，過去には気付かれなかつたような所見が，螢光眼底撮影を含め，少なからず報告されるようになつた^7〜11）。

We followed up a 28-year-old male by means of fluorescein angiography who developed sud-den visual disturbance and who was diagnosedas acute myeloblastic leukemia by peripheral blood examination and bone marrow biopsy. Before initiation of chemotherapy and blood transfusions, both fundi manifested numerous capillary microaneurysms, dilatation and tortuo-sity of retinal veins, and massive retinal hemor-rhages. The microaneurysms and pathological changes of veins disappeared after the leuke-mia was brought under control.
