眼窩内容除去術における出血量と輸血量

Bleeding and Blood Transfusion in Exenteration of the Orbit

緒言

　眼窩内容除去術は，眼科領域において最も積極的な治療法の一つであり，しばしば術中多量の出血を見，輸血を必要とすることもまれではない。しかしながらこの手術の行なわれる機会は少なく，現在まで本邦において眼窩内容除去術を行なつた症例群に関する統計的報告を，いまだ見ない。欧米においては，眼窩内容除去術を行なつた症例群につき，頻度，年齢，病理診断，予後，眼窩再構築術等についての報告を散見するもの^1〜3）の，出血量および輸血に関する記載がない。

　本稿は，1958年より1975年までの18年間に，京大眼科において行なわれた眼窩内容除去術に関する研究の一部である。われわれが最近18年間に行なつた眼窩内容除去術につき術中の出血量，輸血量を記録にとどめ，年齢，術式，何回目の手術かによつて，それらを比較検討し，眼窩内容除去術における術中出血と輸血について論ずる。

Exenteration of the orbit is the only operation in ophthalmology which requires blood trans-fusion. Twenty-two orbital exenterations have been done in the past 18 years at the Depart-ment of Ophthalmology of Kyoto University Hospital. Ten cases were adult at the time of exenteration and 12 cases were children under 10 years of age. In all cases exenteration was done for malignant tumor of the orbit.

The amounts of bleeding at the operation and of blood transfusion were examined at the operation records in all cases.
