Transpupillary Cyclophotocoagulation出血性緑内障の一症例

Transpupillary Cyclophotocoagulation. A Case Report of Haemorrhagic Glaucoma

緒言

　出血性緑内障は治療のきわめて困難な緑内障の一つである。著者は，Argon Laserを用いてTranspupillary Cyclophotocoagulationを行ない，良好な結果を得た出血性緑内障の一症例を経験した。

　この方法が用いられたのは，毛様体ジアテルミーをはじめとした種々の治療にもかかわらず高眼圧と眼痛が持続していたこと，患者は若い女性であり眼球摘出は避けなければならなかつたこと，与える疼痛と侵襲を最小限にしたかつたこと，経過中に虹彩と水晶体の間に間隙が生じ，隅角鏡を装用するとその間隙から毛様体突起が透見可能になつたこと，Lee （1971）^3）のこの方法を用いた実験では，眼圧下降がみられ大きな副作用がなかつたこと，この方法に対する患者の同意が得られたこと，などの理由のためである。

A 21-year-old female developed haemorrha-gic glaucoma in the left eye and complained severe headache and vomiting, despite intensive treatments with systemic acetazolamide and mannitol, topical epinephrine and cyclodiather-my. The pupil became widely dilated with ectropium uveae, permitting direct visualization of the ciliary processes over the entire circu-mference. Argon laser photocoagulation was carried out using Goldmann' s three mirror con-tact lens.
