眼性斜頸とその治療

Diagnosis and Surgical Treatment of Ocular Torticollis

緒言

　眼性頭位異常のうち，垂直筋異常による眼性斜頸は日常しばしばみられ，決してまれなものではないが，わが国では眼性斜頸に関する詳細な報告は少なく^1）2）3），とくに多数例についての検討は行なわれていない。また，斜頸の方向と異常筋との関係についても，種々論議があり，未だ結着がつけられていない現状でもある^4）。しかも，先天性ないし後天性でも乳幼児期に発病した眼性斜頸は頭位異常ばかりでなく，顔面の左右非対称をおこすので，正しい早期診断と，早期治療が要求されねばならない。

　そこで，最近2年間に経験した62例の幼時から存在する眼性斜頸について検討を加えてみた。その結果から現時点での眼性斜頸の診断と治療，および問題点を述べてみたいと思う。

62 cases of the ocular torticollis were exa-mined. It was found as many as 52 cases of them were caused by the palsy of superior oblique muscle.

When the ocular torticollis was investigated by the way of the three-step procedure, the diagnosis was not always the same as when examined by the major amblyoscope and the Hess coordinator.

11 cases of 62 cases with the ocular torticol-lis had been already diagnosed and therapied as the infantile muscular torticollis by the or-thopedists.

The operations of 48 cases with the ocular torticollis were performed.
