全身麻酔下の小児眼圧について（第2報）

Intraocular Pressure in Children under General Anesthesia (Report II)

緒言

　先にわれわれは，シェッツ眼圧計を用い，小児を対象にして局所麻酔下，全身麻酔下の測定値をScale readingにて表示した^14）。しかし，小児に対してシェッツ眼圧計を用いることは，測定技術はいうまでもなく，Ocular rigidityの問題や角膜の性状などが，成人の正常眼と同一であるかどうかという配慮のもとに測定が必要であり，この点でも正しくシェッツ眼圧計の測定値を評価することがむずかしかつた。このためぜひ圧平眼圧計を用いることにより，圧平眼圧計の測定値を求めるとともにOcular rigidityの値を検討する必要があつた。しかし小児に対して圧平眼圧計を用いるには，やはり技術上の問題が多く今回測定に用いた器械の選択までには，かなりの時間を要したが，結局あたらしいタイプの眼圧計として，fast acting application tonometerであるマッケイマーグ眼圧計を選び，他方ゴールドマンタイプの圧平眼圧計に類似したドレーガーの手持圧平眼圧計を用いた。

A total of 68 subjects (97 eyes) between 2 and 13 years of age were put to tonometric studies under topical and general anesthesia just before undergoing squint surgery. SchiOtz and/or Mackey-Marg tonometer was used for the tonometry.

During the third-stage anesthesia induced by endotracheal Flouthane, the ocular tension in 73 eyes of children between 3 and 7 years of age averaged 16. 72 mmHg by 5. 5 g loading with Schibtz tonometer. The ocular tension in 81 eyes from the same age group averaged 18. 2mmHg by Mackay-Marg tonometer. No definite correla-tion was observed between the ocular tension and the age of the subject.
