全身麻酔下の小児眼圧について（第1報）—シェッツ眼圧計による方法

Intraocular Pressure in Children under General Anesthesia

緒言

　顕微鏡手術の導入により，先天緑内障の治療面の進歩は著しいものがあるが，十分な治療効果を得るには，本症の早期発見が基礎となることはいうまでもない。一方，本症の診断は対象が小児であることから，もつぱら他覚的所見によることが多く，就中，初期の診断には眼圧測定が重要である。しかし，小児の眼圧測定にはしばしば全身麻酔が必要となり，眼圧値を容易に測定することは困難であり，また測定し得ても，いわゆる正常値というものが明らかでないために，必ずしも測定値を有効に生かし得なかつた。

　そこで，われわれは当小児保健センターで，外眼手術を要す患児に，全麻下，局麻下の眼圧をシェッツ眼圧計にて測定を行ない，全麻下の小児の平均眼圧測定目盛値を求めるとともに，局麻下での測定値との比較により，全麻下の眼圧への影響などを検討し，結果を得たので報告する。

The intraocular pressure of 770 eyes in child-ren from 1 to 13 years of age was measured by Schiötz tonometer under G.O.F. anesthesia.

The scale reading during general anesthesia at stage III was 5.6±2.2 (mean±s.d.) for 5.5g loading and 11.2±2.9 (mean±s.d.) for log load-ing.

On the other hand, the intraocular pressure of 310 eyes from the same group under topical anesthesia with benoxylate hydroxide was found to be 5.3±1.4 for 5.5g loading and 10.6±1.1 for 10g loading.

The findings indicate that a significant decre-ase in IOP occurs during general anesthesia by GOF.
