中心性網脈絡膜炎における循環動態の検討—容積脈波よりのApproach

Systemic Circulation in Central Serous Retinopathy: A Plethysmographic Evaluation

緒言

　中心性網脈絡膜炎（増田型）の発症部位については，近年細隙灯顕微鏡による精査，螢光眼底写真の発達，および光凝固療法による病巣の変化等により，ほぼ確定した感がある^1）〜5）。しかしその原因については，不明の血管障害によるものとの考え方が有力ではあるが，現在なお不明である。

　今回著者らは，中心性網脈絡膜炎患者の胸部X線写真，および光電式指尖容積脈波（以下脈波と略記）より，興味ある所見を得たので報告する。

　Photoplethysmography of the finger has been tested in 10 patients with central serous retinopathy. The plethysmographic pattern in patients during the early stage of the disease showed anacrotic waves, indicating the presence of cardiac mechanical insufficiency with increased peripheral resistance. Impaired cardiac function was detected in 4 subjects.

　X-ray findings of the chest revealed cardiac insufficiency in all cases. In 8 cases, chronic ob-structive pulmonary lesion such as pulmonary emphysema was detected.
