姉妹に現われた家族性遺伝性黄斑変性

The Vitelliform Macular Degeneration in Three Sisters

緒言

　家族性遺伝性黄斑変性のうちBest型といわれるものは，初期には黄斑部に境界鮮明な卵円型の黄色い病巣を呈するため，一般に卵黄様黄斑変性と呼ばれている^1）。本症はわが国には比較的少なく，湯口の1例^2），塚原の3例^3）4），松井の1例^5），山田の2例^6）等が報告されている。わが国の報告はいずれも散発例で，家族性の報告はみあたらない。今回われわれは3人姉妹のうち2人は卵黄様黄斑変性と思われる病変を示し，1人は卵黄様黄斑変性のごく初期であろうと思われる症例を経験した。そして初発症状は検眼鏡的変化のほとんどない時期から，EOGおよび後極部局所ERGに化が現われてくるのではないかという興味ある所変見をうることができた。

The vitelliform macular degeneration appear-ing in all of the three sisters was studied. The usual electroretinogram (ERG), focal ERG of the macula, electrooculography (EOG) and fluo-rescein photography have been performed. The eldest sister showed a fundus finding similar to that described by Braley as polymorphic macu-lar degennration. The second sister showed a fundus finding and course of the disease cha-racteristic for the scrambled egg stage of vitel-liform macular degeneration.

The youngest sister showed little change by ophthalmoscopy.
