眼外傷の黄斑部に及ぼす影響に関する研究（第1報）—鉄片外傷による猿眼黄斑部の変化

Studies on Experimental Intraocular Siderosis

緒言

　鉄片が眼内に残存すると眼内組織の障害が生じ，臨床的に鉄錆症が発現するが，その障害は眼内に平等に起こるのではなく，部位的に黄斑部が限局性変化を受けやすいことをHaabがはじめて記載して以来，内外ともに多くの報告がなされてきた。

　最近では，牧内氏がサルの硝子体内に鉄片を挿入して実験を行ない，初期のおもな組織的変化はHenle線維層の浮腫，色素上皮細胞の変性，脈絡膜後極部の滲出液貯溜であり，従来の鉄錆症臨床報告例にみられるごとく，黄斑部が限局性に侵されるのを確認した。

Intraocular siderosis was experimentally pro-duced in 4 monkey eyes by inserting a small iron fragment into the vitreous and was follow-ed up by ophthalmoscopy and fluorescein angiography.

Both the ophthalmoscopic and fluorographic findings in normal monkey eyes were essential-ly similar to those in humans.

The onset of ocular siderosis was characte-rized by hyperemia of the optic disc, dilatation of retinal vessels and the retinal edema involv-ing the macular region. With the progression of ocular siderosis, the retinal vessels showed remarkable constriction and further increase of the macular edema.
