眼球圧迫試験（須田）後にみられた前房隅角出血

Haemorrhage from the Schlemm's Canal at the Anterior Chamber, Induced by the Bulbar Compression Test (Suda)

緒言

　シュレム管（S管）に相当する部位に充血の認められることは，Salzmannの記載^1）以来幾多の報告がある^2）。しかしながら，一定の外力を加えてS管に充血を誘発せしめて観察したのは，Kronfeld^3）4）がはじめてである。わが国においても，眼球加圧後のS管の充血現象について，池辺^5）〜7）（10例20眼），大野^8）（47例81眼）の詳細な観察がある。いずれもS管の充血を認めても，血液の漏出を伴つた例は皆無と報告している。

　著者らはPosner Schlossman　の1症例で，S管の位置，幅等の状態を調べるために，須田圧迫試験を施行し，充血現象を誘発せしめたところ，加圧除去後S管より前房内への出血を認めた（Fig．1）。これまで，須田圧迫試験においては，いわゆる定型的な充血現象^3）を認めるのみで，かような出血例の報告がなかつたので，さらに症例を追加して検索した。そして6例7眼にS管よりの出血を認めたので，その出血の状態とその原因と考えられる因子について報告する。

Haemorrhage was observed from the Schle-mm's canal into the anterior chamber after the bulbar compression test (Suda). No case has been reported of haemorrhage after Suda's comp-ression test so far.

The haemorrhage was observed in 4 eyes (4 cases) from 5 cases of Posner-Schlossman synd-rome, 1 eye of heterochromic cyclitis out of 5 cases of uveitis, and 2 eyes (1 case) of ocular sarcoidosis. All eyes showing haemorrhage had secondary glaucoma with high ocular pressure except for a case of ocular sarcoidosis. The ocular pressure after the compression test was not necessarily low in these eyes with haemorrhage.
