汎下垂体機能低下と興味ある眼科的所見の変動を示した嫌色素性腺腫の1例—鼻咽頭伸展を主とした1例

A Rare Course of Chromophobe Adenoma with Nasopharyngeal Extention

緒言

　嫌色素性下垂体腺腫は，組織学的には良性を示すが，しばしば鞍外伸展を示すが故に，所謂悪性下垂体腺腫の形をとる事がある。鞍外伸展を示す下垂体腺腫と鞍内型のそれとでは，その予後に於て両者間の格段の差^1）2）3）があり，このため前者に対する診断治療は，あらゆる面から実に重要な問題を含んでいる。中でもこの鞍外進展型のうち咽頭進展は，非常に頻度は低く，その生じ得る病像もまだ明らかではない。

　今回我々は，嫌色素性腺腫の咽頭伸展形の1例に於て，興味ある経過を示したものを経験したので，報告する。

A report is presented on an unusual case of pituitary chromophobe adenoma extending into the nasopharyngeal region.

A 56-year-old male complained of disturbed vision in Feb. 1967. The presence of bitemporal hcmianopsia was detected two months later. The sellar groove was enlarged on skull x-ray studies.

The patient was referred to the authors and was operated on early July after he had develop-ed slight panhypopituitarism symptoms. A large tumor mass was present in the sella with enormous extension into the surrounding tissue.The tumor proved to be chromophobe adenoma through histological studies.
