意識障害者・小児の視野計測法—赤外線テレビによる瞳孔反射視野計

Pupillary Reflex Perimeter for Children and Unconscious Patients

I．緒言

　現在まで視野計測はすべて患者の光刺激の認知による返答によつて行なわれたため，小児や意識障害者の視野計測は全く不可能であつたが，われわれは患者の返答のかわりに対光反射を指標とすることにより，検査に非協力な患者の視野計測も可能な装置を考按した。

　古く1881年Wilbrand^17），1883年Wernicke^16）は半盲側からの光の投射に対しての対光反射は，その健側に比べて弱いことを記載している。Hess （1907）^9）やG．Braun （1934）^3）らは種々な装置を考按して，そのことを確かめるべく努力した。1949年H．Harmus^8）は対光反射による視野計測を視野内の一経線上ではあるが定量的に計測し，その事実を実証している。これらの研究はすべて細隙灯などの通常の光学系に頼り，手技上多くの煩雑さがあつた。われわれは赤外線テレビジョンを使用することにより，容易に全視野にわたる対光反射による視野計測を行ないうる装置を考按し，臨床例にて十分な成績を得た。

We have developed two kinds of pupillaryreflex perimeter using the infrared television. One for sitting position is intended to compare the results of this method with that of the usual perimeter. Another for supine position is applied for those who are unable to sit, as unconscious patients, children under light ane-sthesia and animals as well.
