糖尿病性網膜症に対する定位的経鼻脳下垂体手術

Stereotaxic Trans-sphenoidal Hypophysectomy of Diabetic Retinopathy

Ⅰ．緒言

　1942年Houssayらが実験的糖尿病動物に下垂体切除術を行なうことにより，糖尿病症状の著明な改善が認められることを報告して以来，下垂体前葉ホルモン，とくにhuman growth hormonの糖尿病源性作用が，注目されるようになつた。臨床的にも，下垂体破壊手術が行なわれるようになり，諸外国において数々の報告がなされている。

　下垂体への到達方法も種々あるが，われわれは定位脳手術法により，鼻孔より下垂体に到達し，高周波凝固による下垂体手術を16例行ない，そのうち13例は糖尿病性網膜症の患者に行なつたので，ここに報告する。

Stereotactic transsphenoidal hypophysectomy has been conducted on 12 cases of diabetic re-tinopathy using our stereotactic apparatus and thermo-controlled radiofrequency coagulator. During the postoperative observation period averaging 12 months, visual acuity improved in 4 cases, enlargement of visual field or de-crease in the size of scotoma set in in 7 cases and the visual function deteriorated further in 2 cases which belonged to the terminal stage of retinopathy already before the operation. No severe postoperative complication has been en-coutered except transient nasal liquorrhea.
