無核白内障（軟性白内障）の吸引法における術式と合併症との関連について

Aspiration of Cataracta Mollis, Evaluation of Three Method as Regards Postsurgical Complications

I．緒言

　無核白内障（軟性白内障）に対する吸引法は，昭和34年に本邦で初めて桑原教授が紹介して以来，比較的手術操作が容易であり，安全で，患者の負担も少なく，また忌むべき合併症も少ないなどの多くの利点から，最近では広く無核白内障に対し利用できる優れた方法として，わが国でもここ10年来，その症例数が年々増加の傾向にある^6）7）9）。

　当教室においても，昭和37年以来，無核白内障に対し，routineに吸引法が行なわれてきたが，この間桑原教授は，漸次術式に工夫改良を加え，現在までに術式に関し，三つの時期的変遷を見ることができる。今回昭和37年4月より，昭和44年3月までの7年間に，無核白内障に対し，これら3期の各術式で水晶体吸引を行なつた124例145眼につき，合併症を中心に統計的分析を試み，あわせて各種の臨床所見につき，術式別に比較検討したので，ここに報告する。

Aspiration of cataracta mollis has been per-formed in 145 eyes of 124 cases during the last 7 years. Three different surgical methods have been devised and practised as routine proceduresin these cases.

These surgical techniques have been evalua-ted as concerns complications during and after the surgery. Simple aspiration (shown as A in Fig. 1 in the text) and aspiration while irrigat-ing the anterior chamber through another nee-dle inserted in the anterior chamber (shown as 13 in Fig. 2 in the text) have proved unsatis-factory as the pupil tended to become miotic during the surgical procedure.
