斜視手術に偶発した網膜中心動脈枝閉塞症について

Occlusion of retinal branch artery complicating strabismus surgery

　術前一般検査で特に異常のない22歳女性の外斜視の手術中術眼に発症した細膜中心動脈枝閉塞症の1例を報告した。

　その閉塞機転として，自律神経系の異常を基礎とし手術時の心理的負荷，疼痛，眼局所への手術操作等が何らかの誘因となり，術眼の網膜中心動脈ことに上黄斑動脈の攣縮を惹起し，一過性視力消失および上黄斑動脈支配領域の永続的な機能障害を残したと考えた。

　さらに外斜手術翌日よりの術眼の視力低下を訴え，第26病日に来科した25歳女性の網膜中心動脈枝閉塞症を経験し，著者らの経験したものと類似の機序による閉塞の可能性もあると考えられたので併せ報告した。

Two cases of retinal artery branch occlusion occuring during and after strabismus surgery are reported. The first case, a 22-year-old female, was free of any contributable past history except for three previous unsuccessful strabismus surgeries. She underwent a fourth operation under local anesthesia with 2% Lidocaine containing epine-phrine with additional sedation by 4 mg diazepam dissolved in 8 ml of physiological saline through intravenous route. Immediately after recovery from general anesthesia, the patient noted blindness of the operated eye.
