重症筋無力症眼球型に関する臨床的研究

Clinical Studies on Ocular Myasthenia Gravis

I．緒言

　重症筋無力症はWillis （1672）により最初に記載された。さらにErb （1878），Goldflam （1893）により詳しく報告され^1），Erb-Goldflam病と呼ばれた。Jolly^2）（1895）はMyasthenia gravispseudoparalyticaと呼び，彼によつて初めて重症筋無力症と名付けられた。

　重症筋無力症の眼症状については，Mattis^3）（1941）やWalsh^4）（1945）らの詳細な記載がある。Gavey^5）（1941）は25年間，眼にのみ症状がとどまつていた例を報告している。眼症状だけの重症筋無力症をWalshは"Ocular"Myasthe—nia gravisと呼び，Lisman^6）（1949）も症状が眼にのみ現われた2例を報告し，Ocular Mya—sthenia gravisと題している。Ocular Myasth—lenia gravisに対峙して，症状が全身に亘るものをSystemic Myasthenia gravisと呼び，臨床的に重症筋無力症の病型を上記の2つに分ける場合がある。

Nine cases of ocular myasthenia gravis we-re reported.

The present cases consisted of four males and five females. The age of onset of the disease ranged from 16 months to 64 years.

Ptosis of one or both eyelids, diplopia, and limitation of ocular movements were common symptoms and in three cases general fatiga-bility of some duration were observed.

Overwork, pulmonary tuberculosis and pal-pebral edema occurring after applying a pla-ster ("Tokuhon") appeared to be the presuma-ble incentives in some cases.
