Desoxyribonucleaseによる角膜ヘルペスの治療

The Clinical Observations of the Effect of Desoxyribonuclease in the Treatment of Herpes Corneae

I．緒言

　1964年ソビエトのK．P．Kaplinaは角膜ヘルペスの治療にDesoxyribonucleaseを用いる方法を報告し，角膜ヘルペスに対する特殊療法に第2の道を開いた。

　さて第1の道は第1表に表示する様に1962年Kaufman等により報告されその後多くの眼科医達により追試された5—iodo−2—desoxyuridine（IDU）或はそれに類似するAntimetabolic Su—bstancesを用いる方法でこの内IDUは我国に於ても市販されて現在迄難治であり，亦抗生物質や，Steroidhormonの出現によつてその病型をも変えて我々を悩ました角膜ヘルペスの治療に光明をもたらした。しかし，IDUはMaxwell等の集計によると表在性の病変のみの症例に対しては極めてすぐれた効果を呈するのに反して，角膜実質にも病変の及んだ症例ではその効果は前者に比較してかなりの差があり，亦，Kaufman自らも述べる様にヘルペス性虹彩炎には無効で円板状角膜炎に対してもその効果には疑問があるとの事である。亦小林氏等の研究によると，ヘルペスウィルスは試験管内で，IDUに対して抵抗性を獲得することが可能で，このIDUに抵抗性を獲得した。

Since Kaufman reported the use of IDU to herpes corneae, this medicine has been widely used ; however, as even Kaufman mentioned in his report, IDU is of striking benefit in dendritic keratitis and geographic ulcers but is not of benefit in herpetic iritis and is of questionable value in disciform keratitis. In the statistics by Maxwell, IDU seems less effective in cases with stromal involvement.Kobayashi observed that herpes virus becomes resistant to IDU experimentally. Thus, it was hoped to find a new agent which is effective for both stromal cases and IDU resistant cases.
