肺癌の眼瞼転移症例と悪性腫瘍の眼瞼転移について

Metastatic Tumors of the Eyelid from Carcinoma of the Lung and Review of the Literatures

I．緒言

　最近，札幌医大眼科に於いて，76歳男性にみられた右肺癌の左下眼瞼転移の珍らしい症例に遭遇したので報告する。本症例では眼瞼癌の診断のもとに入院させ全身検査より胸部レ線写真像に異常陰影を認め，喀痰中に悪性腫瘍細胞を証明し肺癌による陰影である事を確かめて居り眼瞼の放射線治療を行ない，肺癌の治療のため放射線科に転科し2週後に全身衰弱のため病状急変し死亡した。病理解剖により原発巣の大なる肺癌を認め，眼瞼癌は転移巣である事が確められた。原発性眼瞼癌は多数報告されているが悪性腫瘍の眼瞼転移は非常に稀なもので欧文，邦文合わせて現在迄，僅かに20例の報告をみるに過ぎず，その中，肺を原発巣とするものは5例である。"肺癌の眼瞼転移"の報告例としては，本邦に於いては九大眼科村川氏^1）が1963年に1例報告し抄録が眼科臨床医報に掲載されて居り，本症例は第2例目の本邦報告例である。症例の紹介及び悪性腫瘍の眼瞼転移について文献調査を行なつたので報告する。

A case of metastatic involvement of the left lower eyelid from a squamous cell car-cinoma in the right lower lobe of the lung is reported and the literatures of metastases of malignant tumors to the eyelid are reviewed.

A 76-year-old man had a chalazionlike no-dule in his left lower eyelid since 3 months before. The histological examination of the excised specimen recorded squamous cell car-cinoma and he was admitted to the Eye Cli-nic of the Sapporo Medical College Hospital for further examination and treatment.
