黄斑部変性症と視路疾患の鑑別

　眼底の乳頭黄斑部領域における網膜面の異常所見，および，恰もそれに対応する盲斑中心暗点の形，以上のふたつの条件が，昔からわが国のいわゆる「軸性視神経炎」ないし「球後（＝球外）視神経炎」の診断に，特異な価値をもつものとして重視され，現在も一部の人から承認されている^1）〜3）。

　しかし，このふたつの条件は，色々な病気や病的状態のもとで，比較的しばしば組合わされる機会があり，決して，視束疾患の診断に特有な価値をもつものではない^4）5）。

A fifty-five-year-old man is the described case, who had previously normal vision in both eyes, complained of loss of central vision in both eyes and was treated by a clinician as bilateral"axial neuritis"for several years but in vain. The results of author's examination are as following : Each visual acuity of the two eyes was 0.5 meter/finger counting, there was absolute central scotoma of about 10°—15° demonstrable, but the dark adaptation was almost normal ; the fundus lesion was symmetrical in both eyes, the macula region appeared gray-blue, there were irrregular pigmentation and yellow-gray spots.
