手指化膿巣が原発巣と考えられた内因性眼内炎の1例

A case of bacterial endophthalmitis presumably metastatic from infectious foci in fingers

　32歳女性が3週間前からの左眼視力低下で受診した。12年前に全身性エリテマトーデスと診断され，プレドニゾロン内服を続けていた。3年前から腰椎圧迫骨折後の腰痛に対してジクロフェナックを内服していた。3か月前にすべての手の指先に化膿巣が出現した。矯正視力は1.2，左指数弁であり，左眼には前房蓄膿と硝子体混濁があった。白血球増加や肝膿瘍などの全身異常はなかったが，両指先に化膿巣があった。採取された硝子体と手指から，薬剤感受性が等しいメチシリン感受性黄色ブドウ球菌（MSSA）が検出され，転移性細菌性眼内炎と診断した。易感染状態にある場合には皮膚感染巣でも眼内炎の原発巣になりえること，消炎鎮痛薬の内服中では眼内炎の症状が出現しにくいこことを示す症例である。

Purpose：To report a case of endophthalmitis metastatic from infectious foci in the fingers. Design：Interventional case report. Case：A 32-year-old female persented with visual loss in her left eye since 3 weeks before. She had been receiving peroral prednisolone for systemic lupus erythematosus since 12 years before. She had been receiving systemic diclofenac for backache after compression fracture in the lumbar vertebra. She had had abscess in the fingertips since 3months before. Her visual acuity was 1.2 right and counting fingers left. Her left eye showed hypopyon and vitreous opacity. Results：Culture of vitreous aspirate showed methicillin-sensitive Staphylococcus aureus（MSSA）. The same strain was isolated form the fingertips. These findings led to the diagnosis of metastatic endophthalmitis. Conclusion：Even infectious foci in the fingertips may lead to metastatic endophthalmitis,particularly in an immunocompromised patient.
