先天性涙囊ヘルニアに対して18ゲージ針を用いた造袋術を行い治癒した1例

A case of congenital dacryocystocele treated by puncture with an 18-gauge needle

要約　目的：先天性涙囊ヘルニアに対し，18ゲージの注射針で穿刺し，良好な結果が得られた症例の報告。症例：在胎37週で自然分娩で生まれた女児に，出生時から左眼内側に10mm×5mmの青灰色の腫瘤があった。先天性涙囊ヘルニアと診断した。生後5日目に，耳鼻科で鼻内鏡による直視下で，18ゲージ注射針で囊腫を切開し，造袋術を行った。その直後から腫瘤は消失し，以後1か月間，再発はなかった。結論：新生児の先天性涙囊ヘルニアに対し耳鼻科医の協力を求め，鼻腔鏡による直視下で穿刺を行い，治癒が得られた。

Abstract.　Purpose：To report dacryocystocele in a newborn baby treated by puncture with an 18 G needle. Case：A girl was born after gestation of 37 weeks. She showed a tumor of 5 mm by 10 mm in size nasal to the inner canthus in the left eye. She was diagnosed with congenital dacryocystocele and was referred to a rhinologist. The cyst was punctured by an 18 G needle under direct observation by nasal endoscope. The cyst disappeared immediately with no further recurrence. Conclusion：Dacryocystocele in a newborn was treated by puncture under direct observation with a rhinoscope.
