Abstract. Purpose:To report a case of optic neuritis seropositive for anti-aquaporin-4 antibody with clinical features simulating anterior ischemic optic neuropathy. Case:A 37-year-old woman presented with pain and impaired vision in her right eye since 3 days before. Findings:Corrected visual acuity was 0.07 right and 1.2 left. The right eye showed optic disc edema,tortuous retinal vessels,upper horizontal hemianopia,and caecocentral scotoma. Magnetic resonance imaging showed high-signal optic nerve. Fluorescein angiography showed prolonged intraretinal circulation time. These findings led to the diagnosis of optic neuropathy associated with anterior ischemic optic neuropathy. The right eye recovered one month later after pulsed corticosteroid therapy. She was later found seropositive for anti-aquaporin-4 antibody. Conclusion:Anti-aquaporin-4 antibody may have contributed to the unusual clinical manifestations of optic neuritis in this patient.
