潰瘍性大腸炎に併発したカンジダ角膜炎の1例

Candida keratitis in a case of ulcerative colitis

要約　目的：カンジダ角膜炎が経過中に発症した潰瘍性大腸炎の症例の報告。症例と経過：68歳男性が潰瘍性大腸炎と診断され，プレドニゾロン内服と中心静脈栄養（IVH）が行われた。治療開始から5週間後に血液培養でCandida albicansが検出された。両眼に角膜後面沈着物があり，前部ぶどう膜炎と診断した。ステロイド剤の点眼と抗真菌薬の全身投与で眼所見は寛解した。11か月後に膿瘍と前房蓄膿を伴う角膜潰瘍が右眼に発症した。擦過物からC. albicansが検出された。抗真菌薬の点眼と内服で6週間後に瘢痕治癒した。結論：カンジダ血症では，軽症の前眼部ぶどう膜炎が長期間続いたのちに真菌性角膜炎が生じうることを本症例は示している。

Abstract.　Purpose：To report a case of ulcerative colitis who developed candida keratitis during the course of treatment. Case and Findings：A 68-year-old male was diagnosed with ulcerative colitis and was treated by peroral prednisolone and intravenous hyperalimentation. Candida albicans was detected by culture of systemic blood 5 weeks after start of treatment. Both eyes showed signs of mild anterior uveitis. Ocular findings showed remission after topical prednisolone and systemic antifungal drugs. Corneal ulcer with abscess and hypopyon developed 11 months later. Candida albicans was deteced in the smear from the ulcer. Topical and systemic antifungal drugs resulted in cure 6 weeks later. Conclusion：This case illustrates that candidemia may manifest mild anterior uveitis initially to be followed by fungal keratitis after a long interval.
