Abstract. Purpose:To report the incidence of ocular manifestations and related factors in patients of Graves disease before treatment. Cases and Method:This study was made on 171 patients who were referred to us under diagnosis of Graves' disease. The series comprised 26 males and 145 females. The age ranged from 14 to 74 years, average 38 years. Cases were eliminated who were diagnosed with euthyroid Graves disease or who had had ocular symptoms for 3 months or longer. They were examined regarding thyroid stimulating hormone(TSH), TSH binding inhibitor immunoglobulin(TBⅡ), thyroid stimulating antibody(TSAb), thyroglobulin antibody(TgAb), and thyroid peroxidase antibody(TPOAb). They also received magnetic resonance imaging(MRI)regarding the cross area of extraocular muscles and T2 signal intensity ratio(T2SIR). Results:Eye symptoms were present in 78% of cases. The positive rate was 98.2% for TBⅡ, 86.5% for TSAb, 53.2% for TgAb, and 66.7% for TPOAb. Proptosis was related with lower age and the value of TSAb. Total value of coronal section of extraocular muscles was related with the level of TSAb, history of smoking, higher age, and male gender. Conclusion:Ophthalmopathy may already be present at a high rate in patients of Graves disease before treatment. Its detection may be facilitated by MRI and fluctuation of TSAb.
