Abstract. Purpose:To report a case of intraocular lymphoma that responded to vitrectomy and intraocular methotrexate(MTX). Case:A 50-year-old woman presented with myodesopsia in her left eye since one month before. Findings:Corrected visual acuity was 1.2 in either eye. The left eye showed vitreous opacity with floaters. She was diagnosed with uveitis. Topical corticosteroid was followed by improvement. Left visual acuity decreased to 0.7 15 months later. Corticosteroid was again effective for vitreous opacity. Both eyes showed vitreous opacity another 15 months later. The right eye showed retinal patches in the periphery. The left eye received vitrectomy leading to the diagnosis of malignant lymphoma. The right eye was treated by vitrectomy followed by intravireal MTX for 10 months. Both eyes showed improvement. Intracranial lesion developed 3 years later and was treated by radiation. Conclusion:Vitreous surgery was effective for vitreous opacity due to malignant lymphoma. Retinal infiltration responded to intravitreal MTX.
