We performed vitreous surgery for diabetic macular edema in 28 eyes of 21 patients. We paid particular at-tention to postoperative changes in the number of capillary microaneurysms. The series comprised 17 males and 11 fe-males. Their ages ranged from 40 to 67 years, average 60 years. Macular edema disappeared in all the eyes 1 to 18 months, average 4.6 months, after surgery. We counted the number of microaneurysms within 3 disc diameters from the fovea on fluorescein angiogram. It averaged 72±40 before surgery and 52±33 after surgery. The difference was significant (p<0.001). There was a positive correlation between the reduction in the number of microaneurysms and the postoperative duration of macular edema. Reduction in the number of microaneurysms was more pronounced in fe-males than in males and in patients more advanced in age. The observed reduction in the number of microaneurysms appeared to he the result of improved background retinopathy consequent to vitreous surgery.
