Abstract. Purpose:To report a case of idiopathic orbital inflammation with suspected pupillary block by pressure to the eyeglobe. Case:A 60-year-old male presented with swelling and hyperemia in the left eye since 12 days before. Findings and Clinical Course:Visual acuity was 1.2 right and 0.6 left when corrected by -3.5 diopters. Intraocular pressure(IOP)was 16 mmHg right and 22 mmHg left. The left eye showed shallow anterior chamber. Magnetic resonance imaging(MRI)showed a soft mass posterior temporal to the eyeglobe suggesting orbital tumor or inflammation. Laser iridotomy resulted in deepening of anterior chamber with decrease in IOP in the left eye. The patient was diagnosed with idiopathic orbital inflammation in another institution 7 days later. Pulsed corticosteroid therapy induced clinical cure with left visual acuity of 1.5 when corrected by -1.5 diopters. Conclusion:Orbital inflammation may have induced pupillary block by pressure to the eyeglobe in the present case.
