Bilateral pinhole法が診断に有効であった3例

Effectiveness of bilateral pinhole method in the diagnosis of malingering or psychogenic visual disturbance

要約　目的：通常の視力検査表と検眼レンズセットで行うことができ，片眼の心因性視力障害や詐病の診断に有効とされているbilateral pinhole法を，片眼の視力障害3例に行った報告。症例：対象は，心因性視力低下が疑われた10歳男児と18歳女性，および詐病が疑われた44歳男性である。罹患眼の視力はそれぞれ0.2，手動弁，光覚弁で，諸検査の結果と視力とが一致しなかった。各症例とも通常の視力検査やトリック法では良好な視力が得られなかったが，bilateral pinhole法で良好な視力が得られ，診断が確定した。結論：原因不明の片眼性視力障害に対するbilateral pinhole法は，心因性視力障害または詐病の診断に有効であった。

Abstract.　Purpose：To report 3 cases of unilaterally impaired visual acuity tested by bilateral pinhole method. Cases：The present a cases comprised a 10-year-old boy and an 18-year-old female who were suspected of psychogenic visual impairment,and a 44-year-old male suspected of malingering. Best-corrected visual acuity in the affected eye was 0.2,hand motion and light perception respectively. After findings by ophthalmological examinations failed to match the visual acuities,full visual acuity was obtained by bilateral pinhole method. Conclusion：Bilateral pinhole method was useful in the diagnosis of psychogenic visual impairment or malingering.
