A 37-year-old man presented with bilateral photophobia since 2 months before. His corrected visual acuity was 0.4 right and 0.8 left. Both eyes showed optic atrophy and constricted visual field. There was no abnormal finding by computed tomography(CT)and magnetic resonance imaging(MRI)in the brain or paranasal sinus. The visual acuity deteriorated to 0.06 right and 0.04 left 8 months later. Serological tests were strongly positive for syphilis. Cerebrospinal fluid showed highly elevated values for Treponema pallidum hemagglutination assay(TPHA)and pleocytosis,leading to the diagnosis of neurosyphilis. In spite of systemic treatments with penicillin G and prednisolone,the visual acuity became hand motion in either eye one year later. This case illustrates that optic atrophy may be the initial manifestation of neurosyphilis.
