シリコーンプローブを活用した涙小管閉塞治療

Treatment of lacrimal canalicular obstruction by silicone probe

要約　目的：ヌンチャク型シリコーンチューブ（NST）とその付属ブジーを一体化したプローブを使った涙小管閉塞治療の報告。対象と方法：鼻涙管閉塞がない涙小管閉塞患者17例20側を対象とした。シリコーンプローブで閉塞部の穿破のあとNSTを挿入し，約2か月後に抜去した。一部に涙小管トレパンまたは金属ブジーを併用した。結果：19側（95％）でNSTを挿入できた。涙小管トレパンを5側，金属ブジーを2側に使用した。NSTの抜去後平均6か月の観察で17側では通水良好であり，再閉塞が2側に生じた。うち1側ではシリコーンプローブで再度穿破を行い，通水良好になった。結論：今回の方法で，涙小管閉塞に対し低侵襲で高い成功率が得られた。

Abstract.　Purpose：To report treatment of canalicular obstruction using a probe attached to nunchaku-style silicone tube（NST）. Cases and Method：A total of 20 sides in 17 patients were treated by this method. NST was inserted after perforation of the obstructed site by a silicone probe. NST was removed 2 months later. Some cases were also treated by canalicular trephining or metal bougie. Results：NST was successfully inserted in 19 sides（95％）. Five sides received canalicular trephining and metal bougie was used in 2 sides. Passage of tear fluid was patent in 17 sides during the follow-up for 6 months after removal of NST. Canalicular obstruction recurred in 2 sides. One side was perforated using a silicone probe and resulted in patent lacrimal passage. Conclusion：The present method induced passage of obstructed canaliculus at a high rate of success with minimum insult.
