ディスポパクレンを用いた結膜弛緩症手術の短期成績

Short-term outcome of surgery on conjunctivochalasis using disposable cautery

要約　目的：使い捨て焼灼器（ディスポパクレン）を使った結膜弛緩症手術の短期経過の報告。対象と方法：手術を行った結膜弛緩症14例16眼を対象とした。平均年齢は73歳である。点眼麻酔のあと開瞼器を使用し，上方視をさせ，下方の弛緩した結膜を眼内レンズ（IOL）鑷子で把持し，ディスポパクレンで弧状に焼灼した。ディスポパクレンには先端温度が450～650℃になるOPTEMPIIV（日本アルコン）を使用した。結膜弛緩症の重篤度は，0～3までの4段階に分類した。術後1か月後の自覚症状と，3か月後までの合併症を評価した。結果：結膜弛緩症の重篤度は術後1週間から改善し，1か月後に0となり，3か月後まで持続した。焼灼した結膜部位は，全例で強膜と接着していた。結論：使い捨て焼灼器（ディスポパクレン）と眼内レンズ（IOL）鑷子を使った結膜弛緩症手術は，点眼麻酔のみによる外来手術が可能で，早期に改善が得られ，格別な合併症がなかった。

Abstract.　Purpose：To report the short-term outcome of surgery on conjunctivochalasis using disposable cautery. Cases and Method：This study was made on 16 eyes of 14 patients who received surgery for conjunctivochalasis. The age averaged 73 years. After topical anesthesia and insertion of lid speculum,the patient was asked to look upwards. The exposed conjunctiva in the lower sector was grabbed by forceps for intraocular lens implantation and was cauterized along an arc. A disposable cautery,OPTEMPIIV（Alcon Japan）,was used that reaches 450 to 650℃ while in use. The severity of conjunctivochalasis was graded into grade 0 to 3. Cases were followed up for 3 months after surgery. Results：All cases started to improve after one week of surgery and reached grade 0 one month later. The conjunctiva adhered to the sclera at the site of surgery. Conclusion：Surgery of conjunctivochalasis was easy,safe and effective with the use of disposable cautery in the present series.
