眼内レンズ脱臼を生じたアトピー性皮膚炎の3例

Luxation of intraocular lens in three cases of atopic dermatitis

要約　目的：眼内レンズ（IOL）が脱臼したアトピー性皮膚炎3症例の報告。症例と所見：症例はいずれも男性で，幼児期からアトピー性皮膚炎があり，掻痒感のため顔面を殴打する癖があった。年齢はそれぞれ31，34，38歳である。いずれも両眼に白内障手術と眼内レンズ挿入術を過去に受け，2例2眼には網膜剝離の既往があった。IOL脱臼は挿入から5年，9年，16年後に生じ，2例では片眼性，1例では両眼性であった。IOLは2眼では硝子体内，2眼では前房内または虹彩前に脱臼した。IOL脱臼に対しては硝子体手術が行われ，2眼ではIOLが再挿入された。結論：網膜剝離手術，白内障手術とIOL挿入の既往があり，アトピー性皮膚炎のために顔面を殴打する癖がある症例では，白内障手術から長期間を経過した後に眼内レンズが脱臼することがあるので注意を要する。

Abstract.　Purpose：To report 3 cases of atopic dermatitis who developed luxation of intraocular lens（IOL）. Cases and Findings：All the 3 cases were males and were aged 31,34 and 38 years respectively. All had atopic dermatitis and habitually struck the face for itchiness. All the cases had received cataract surgery with IOL implantation in the past. Two had received surgery for retinal detachment. IOL luxation occurred 5,9 and 16 years respectively after implantation. Luxation was unilateral in 2 cases and bilateral in one. Luxation occurred into the vitreous in 2 eyes and anteriorly in 2. IOL luxation was treated by vitreous surgery in all the cases. Two eyes received reinsertion of IOL. Conclusion：IOL luxation may develop after years of cataract surgery in cases of atopic dermatitis who have a history of retinal detachment and who have the habit to strike the face for itchiness. Precaution is necessary about IOL luxation in such high-risk cases.
