Abstract. Purpose:To report a family of Wagner disease with cataract and equatorial degeneration or retinal tear. Cases:A 43-year-old female presented with impaired visual acuity in both eyes. Corrected visual acuity was 0.3. Dense nuclear cataract was present in both eyes. Visual acuity improved to 1.2 in either eye after surgery. Both eyes showed multiple equatorial degenerations. Her 17-year-old son had had retinal detachment. Her two daughters,aged 11 and 13 years,showed vitreal liquefaction and equatorial degeneration. Her mother,aged 76 years,had dense nuclear cataract. Visual acuity improved to 0.6 right and 0.9 left after surgery. The right eye showed multiple horseshoe tears in the periphery. The left eye showed equatorial degeneration. These findings led to the diagnosis of Wagner disease of this pedigree. Conclusion:Dense nuclear cataract at a relatively young age may be a sign of Wagner disease. Associated fundus lesion may facilitate the diagnosis.
