バックル除去に至った原因の検討

Causes necessitating removal of scleral buckle after retinal detachment surgery

要約　目的：網膜剝離または未熟児網膜症に対する強膜陥凹手術後にバックル除去が必要となった理由の報告。対象：過去8年6か月間にバックル除去が行われた88眼を診療録に基づいて検索した。結果：手術からバックル除去までの平均期間は，シリコーンラバー24眼では6年1か月，シリコーンスポンジ13眼では2年11か月，マイラゲル^®51眼では12年5か月であった。主な除去原因は，バックルの露出・隆起・偏位と眼球運動障害であり，材料による差はなかった。バックル除去後の合併症として，硝子体内脱出が1例，網膜剝離の再発が2例にあった。結論：マイラゲル^®によるバックル手術では，症状が出現したあと早期にバックル除去を必要とする。シリコーン手術でも長期観察が望ましい。

Abstract.　Purpose：To review the causes of removal of buckling material for retinal detachment or retinopathy of prematurity. Cases：This retrospective study was made on 88 eyes that necessitated removal of buckling material during the past 8 years 6 months. Results：Interval between initial surgery and removal of buckling material averaged 6 years 1 month in 24 eyes receiving silicone rubber,2 years 11 months in 13 eyes receiving silicone sponge,and 12 years 5 months in 51 eyes receiving MIRAgel^®. Removal was necessary because of extrusion,migration or subconjunctival bulging of buckling material,or restriction of eye movement. Intrusion into vitreous occurred in one eye and retinal redetachment in 2 eyes after removal. Conclusion：Early removal of buckling material is necessary for complications by MIRAgel^®. Long-term follow-up is advocated after scleral buckling using silicone material.
