Abstract. Purpose:To report the outcome of surgery for exfoliation glaucoma with subluxated lens. Cases and Method:This study was made on 4 eyes of 4 males treated during the past 2 years. Their age ranged from 67 to 90 years, average 82 years. The visual acuity ranged from 0.05 to 0.4. The intraocular pressure(IOP)ranged from 13 to 28mmHg, average 21mmHg, under medication. Each eye received intracapsular lens extraction through self-sealing wound, anterior vitrectomy, intraocular lens implantation, and trabeculotomy through the corneal wound. Cases were followed up for 3.5 to 8 months, average 6 months. Results:IOP improved to 17mmHg or less in 3 eyes without and in one eye with medication. Visual acuity ranged from 0.3 to 1.2 after surgery. No case showed progression of visual field defect. Transient vitreous hemorrhage occurred in all the eyes. One eye showed detachment of Descemet membrane with decreased population of corneal endothelial cells. Conclusion:The present surgical approach promises to be effective for exfoliation glaucoma with subluxation of crystalline lens.
