眼MALTリンパ腫の5例

Five cases of ocular mucosa-associated lymphoid tissue lymphoma

目的：眼の粘膜関連リンパ組織（MALT）リンパ腫5症例の報告。症例と所見：3例が女性，2例が男性，3例が両眼性，年齢は33，37，39，55，62歳であった。眼以外の臓器に転移はなかった。初診時の症状は流涙，充血，そう痒感，結膜浮腫などであり，2例はアレルギー性結膜炎として加療中であった。結膜の生検を行い，免疫染色で全例が小型から中型のB細胞型の軽症MALTリンパ腫と判定された。放射線療法で腫瘍が縮小した。結論：眼のMALTリンパ腫は結膜または眼瞼の腫瘤やアレルギー性結膜炎に類似した症状を呈することがある。生検が確定診断に有用である。

Purpose：To report five cases of mucosa-associated lymphoid tissue lymphoma of the eye. Cases and Findings：The present series comprised 2 males and 3 females. Three cases were bilaterally affected. They were aged 33, 37, 39, 55 and 62 years respectively. There was no involvement of extraocular tissue. Initial subjective symptoms included epiphora, hyperemia, itching, or conjunctival edema. Two cases had been treated as allergic conjunctivitis. Following biopsy of conjunctiva and immune staining, all cases were diagnosed with low grade, small to medium type of B cell MALT lymphoma. The tumor became smaller following radiotherapy. Conclusion：MALT lymphoma of the eye may simulate tumor of conjuntiva or eyelid, or allergic conjunctivitis. Biopsy is essential for definitive diagnosis.
