長期間経過観察することができた網膜細動脈瘤の1例

Long-term course of a case of retinal macroaneurysm

目的：両眼に発症した網膜細動脈瘤の長期観察例の報告。症例：66歳女性が急激な左眼の視力低下で受診した。矯正視力は右1.2，左0.09であり，左眼の黄斑部に網膜下出血，網膜内出血，網膜前出血があった。網膜細動脈瘤を疑い，フルオレセイン蛍光眼底造影を行ったが，出血のため細動脈瘤は確認できなかった。右眼には上耳側に網膜動脈を囲む白濁が2か所あった。その後左眼の出血が吸収され，網膜細動脈瘤であることが確認された。6年間の経過観察中に右眼の白濁部に細動脈瘤が生じたが，破裂などはなく，右1.0，左0.6の最終視力を維持している。結論：病型と時期が異なる網膜細動脈瘤が両眼に発症することがある。

Purpose：To report the long-term course of a bilateral case of retinal macroaneurysm. Case：A 66-year-old female presented with suddenly impaired vision in her left eye. Her corrected visual acuity was 1.2 right and 0.09 left. Her left fundus showed subretinal, intraretinal and preretinal hemorrhage in the macular area. Fluorescein angiographic findings were inconclusive due to hemorrhagic mass. Her right eye showed opaque patches along the superior temporal artery. The left eye was diagnosed with retinal macroaneurysm after resolution of hemorrhage. During the follow-up for 6 years, a retinal macroaneurysm formed at the site of opaque patch. It showed no sign of rupture. The patient is doing well with visual acuity of 1.0 right and 0.6 lelft. Conclusion：Retinal macroaneurysm may involve both eyes manifesting asymmetrical features.
