視野障害と眼球運動障害を呈したMELASの1症例

Hemianopia and ophthalmoplegia in a case of mitochondrial encephalomyopathy,lactic acidosis,and stroke-like episodes（MELAS）

要約　42歳男性に倦怠感，遠近感喪失，失調性歩行，言語障害が起こり，3週間後に右片麻痺が生じた。10年前から糖尿病があった。両側眼瞼下垂，全方向の眼球運動制限，右同名半盲があり，視力は良好であった。磁気共鳴画像検査（MRI）で，左側頭葉から後頭葉にかけて病巣があった。遺伝子検査でミトコンドリアDNAに点変異があり，さらに筋病理所見，脳卒中様症状と高乳酸血症などからMELAS（mitochondrial encephalomyopathy，lactic acidosis，and stroke-like episodes）の診断が確定した。

Abstract.　A 42-year-old male developed lassitude,loss of sense of distance,unstable gait and difficulty in speech,followed by right hemiplegia 3 weeks later. He had had diabetes mellitus since 10 years before. Ophthalmological examinations showed bilateral blepharoptosis,complete ophthalmoplegia and right homonymous hemianopia. Magnetic resonance imaging（MRI）showed lesions in left occipital and temporal lobes. He had elevated lactic acid in the blood,ragged-red fibers by skeletal muscle bipsy,and point-mutation of mitochondrial DNA. These findings led to the diagnosis of mitochondrial encephalomyopathy,lactic acidosis,and stroke-like episodes（MELAS）.
