Abstract. Sudden headache,nausea and vomiting in a 19-year-old female led to the detection of elevated blood pressure at 250/168mmHg. When seen 2 weeks later,her corrected visual acuity was 1.0 right and 0.05 left. Both eyes showed numerous white spots in deeper retinal layer and serous retinal detachment around the disc and posterior fundus. Fluorescein angiography showed multiple points of hyperfluorescence with dye leakage into the subretinal space. The findings were suggestive of hypertensive choroidopathy. Laboratory studies showed elevated plasma levels of renin activity and aldosterone. She was diagnosed with renovascular hypertension secondary to fibromuscular dysplasia. She received percutaneous transluminal renal angioplasty 6 weeks after onset. The blood pressure spontaneously improved to 130/80mmHg followed by improvement of fundus lesions. The observed choroidopathy was interpreted as manifestation of choroidal ischemia secondary to accelerated arterial hypertension.
