Abstract. Purpose:To report a case of reversible posterior leukoencephalopathy syndrome(RPLS)with bilateral visual impairment as the initial manifestation. Case:A 39-year-old woman underwent extirpation of uterus for malignancy. She developed headache,hypertension,impaired consciousness,and impaired visual acuity 7 hours after receiving anticancer drugs. Her corrected visual acuity was counting fingers in either eye. Both eyes showed central scotoma. Magnetic resonance imaging(MRI)showed high-signal area in the occipital lobe. Antihypertensive and pulsed corticosteroid therapy was followed by improved visual acuity to 1.5. Visual field and MRI findings became normal. These findings led to the diagnosis of RPLS. Conclusion:Impaired visual acuity and visual field may be the initial manifestations in RPLS.
