シリコーンチューブを用いた網膜下血腫移動術

Surgical displacement of subretinal hematoma using an instrument attached with silicone tube

　陳旧性の網膜下血腫4例4眼に対して，新しい手術器具を使って血腫移動術を行った。原因疾患は加齢黄斑変性症3例と網膜細動脈瘤1例で，年齢は78～91歳，すべてが女性であった。器具の先端にはJ字型のシリコーンチューブがついており，網膜上からなでるようにして血腫の移動を試みた。全症例で血腫は移動ないし減少した。視力は2例で向上し，2例では不変であった。術中，器具の操作による網膜裂孔の発生はなく，術後の重篤な合併症はなかった。本法は比較的安全で有効な術式であると結論される。

　We used a new surgical instrument in order to displace long-standing subretinal hematoma in 4 eyes of 4 patients. The hematoma was due to age-related macular degeneration in 3 eyes and to retinal macroaneurysm in one eye. All were females aged fom 78 to 91 years. A J-shaped silicone tube is attached to the tip of the instrument. By gently stroking the retina with the tube,the hematoma could be displaced or diminished in all the 4 eyes. The visual acuity improved in 2 eyes and remained unchanged in 2. No retinal break developed during surgery. There was no serious postoperative complication. We conclude that this method is relatively safe and effective.
