 We observed retinal detachment with giant tear in 14 eyes of 14 patients during the past 5 years. We evaluated the fellow eyes during the follow up of 7 to 132months,average 35months. The series comprised 13males and 1 female. Their age ranged from 6 to 54 years,average 28 years. Retinal detachment with giant tear developed in the fellow eye in 8 cases(57%). It developed within one year after the first eye in 5 cases. Bilateral giant tear occurred in 4 cases,comprising atopic dermatitis 1 case,Marfan syndrome 1 case,pseudophakia 2 cases,and lattice degeneration 1 case. In the other 4 cases with no giant tear in the fellow eye,the retinal break was located in the ciliary body in 3 cases and had horseshoe tear in 1 case. As predisposing factors,2 cases had atopic dermatitis and one had had cataract surgery in the fellow eye. Out of 6 cases that did not develop retinal detachment throughout,one had atopic dermatitis. Retinal detachment with giant tear in our series was characterized by a high incidence of retinal detachment in the fellow eye at the high incidence of 57%and by a tendency for oral or ciliary tear.
