片眼の眼瞼浮腫で発症した甲状腺眼症の1例

A case of dysthyroid ophthalmopathy with unilateral eyelid edema as the initial manifestation

　39歳女性が左上眼瞼の腫脹を主訴に近医を受診し，ステロイド眼軟膏，抗生剤眼軟膏，抗アレルギー点眼薬を処方された。2か月経過したが改善しないため当科を受診した。初診時，左上眼瞼に浮腫状の腫脹を認めた。磁気共鳴画像検査（MRI）では悪性リンパ腫が疑われたため，左上眼瞼の生検を施行した。しかし病理組織学的に異常はなかった。初診から2か月後の再診時に，左眼にGraefe徴候，Dalrymple徴候を認め，甲状腺眼症を疑い血液検査を施行した。Free T₃とfree T₄は高値，TSHは低値であり，Basedow病と診断された。片眼の眼瞼腫脹の鑑別診断として甲状腺疾患も常に念頭に置く必要がある。

A 39-year-old woman presented with swelling of the left upper eyelid. She had been treated,with no apparent effect,by topical corticosteroid,antibiotics and antiallergic agents since 2months before. Magnetic resonance imaging（MRl）showed findings suggestive of malignant lymphoma. Biopsy gave negative findings. Two months later,she developed Graefe and Dalrymple signs,suggesting dysthyroid ophthalmopathy. Hematological studies showed very low TSH,and elevated free T3 and T4 values. These findings led to the diagnosis of Basedow disease. This case illustrates that unilateral swelling of the eyelid may be the initial manifestation of thyroidopathy.
