A 39-year-old woman presented with swelling of the left upper eyelid. She had been treated,with no apparent effect,by topical corticosteroid,antibiotics and antiallergic agents since 2months before. Magnetic resonance imaging(MRl)showed findings suggestive of malignant lymphoma. Biopsy gave negative findings. Two months later,she developed Graefe and Dalrymple signs,suggesting dysthyroid ophthalmopathy. Hematological studies showed very low TSH,and elevated free T3 and T4 values. These findings led to the diagnosis of Basedow disease. This case illustrates that unilateral swelling of the eyelid may be the initial manifestation of thyroidopathy.
