Purpose:To establish a diagnostic criteria to detect spasm of accommodation in myopic school-children by instillating tropicamide ophthalmic solution. Cases and Method:This study involved 86 schoolchildren between 6 and 12 years of age. All were myopic,ranging from -1.25D to -6.25D. We used Mydrin P(R) which contains 0.5% tropicamide and 0.5% phenylephrine. Power of refraction was measured by subjective and objective means before and after intillation. Results:After instillation,myopia decreased by 0.17±0.32D when measured by red-green duochrome test and by 0.30±0.27D by infrared automated refractometer. Conclusion:In order to diagnose pseudomyopia with spasm of accommodation,the power of myopia measured by subjective refraction has to be reduced by more than 0.8 diopters,or 95% confidence interval observed in myopic schoolchildren.
