結膜弛緩症による流涙症への球結膜固定術の検討

Treatment of conjunctivochalasis with epiphora by suturing of conjunctiva to the sclera

流涙の原因が結膜弛緩症に基づく症例があると考え，流涙症を訴え，涙液分泌量が正常もしくは減少しており，鼻涙管が正常に機能している患者のうち点眼により改善のみられない5例5眼に対し，下方結膜を伸展しナイロン糸で3方向を強膜に固定する方法を施行した。全例で術後の涙液メニスカスは正常化し，5眼中4眼で流涙の症状は改善された。手術侵襲や今後の手術への影響を考えると，結膜弛緩症手術の第一選択として結膜固定術は最適であると思われるが，長期経過では効果が不十分である可能性が考えられる。

Conjunctivochalasis may be the underlying cause of epiphora. We sutured the lower portion of bulbar conjunctiva onto the sclera in 5 eyes. All the cases showed epiphora,normal or decreased tear secretion,normal nasolacrimal duct function,and failed to respond to topical medication. The tear meniscus along the lower palpebral margin became normal in all the eyes. Epiphora decreased in 4 eyes. The present method may be useful for conjunctivochalasis with epiphora.
