A 66-year-old male presented with diabetes mellitus detected 18 years before. The blood sugar had been poorly controlled by diet therapy. His visual acuity was 1.0 right and 0.6 left. Both eyes showed retinal hemorrhages. The left eye showed soft and hard exudates. Fluorescein angiography showed dye leakage in the posterior fundus area. He was treated by photocoagulation in both eyes. Macular edema developed 14 weeks later. Visual acuity decreased to 0.6 right and 0.15 left. Vitreous surgery and intraocular lens implantation induced temporary visual improvement. Instillation of nipradilol twice daily was started after visual acuity decreased to 0.1 right and 0.2 left. Macular edema resolved 11 months later. Visual acuity stabilized at 0.2 right and 0.3 left. The HbA1c level remained under 6.6% throughout the course of the disease. This case illustrates that topical nipradilol is effective for diabetic macular edema,possibly through improvement of retinochoroidal circulation,suppression of apoptosis,or activation of cellular functions.
