小指外転筋移行による母指対立再建術の経験—母指形成不全への応用

Opponence plasty by abductor digiti minimi: Application to hypoplastic thumb

緒言

　母指形成不全には種々の程度のものが存在し，その程度により治療法が異なる^6,7）．Blauth^1）は母指形成不全を障害の程度により5度に分類している．Blauth分類4，5度のぶらぶら母指と完全欠損には母指化術が適応になる．また，母指球筋が欠損する2度のものに対しては対立運動の再建が必要である．各種の母指対立再建法の中で，Huber^{4）あるいはLittler5）が報告した小指外転筋移行による方法は，対立運動の再建と同時に母指球のふくらみが形成できる点が特徴である．}

　著者らは，母指形成不全の5例に対して小指外転筋移行による母指対立再建術を行つてきた．その際，原法に改良を加えることによつて満足すべき結果を得ている．原法の手術術式の問題点と著者らが行つた改良点に検討を加えて報告する．同時に，I-II指間の形成あるいは母指MP関節の安定性獲得手術を合併していくことの重要性を強調し，その術式にも検討を加える．

　The standard opponens plasty gives a good functional result but does not restore the cosmetic appearance of the thenar eminence. Five cases of hypoplastic thumbs were treated by Littler's hypothenar muscle transfer, in order to restore opposition of the thumb and reform the wasted thenar eminence. Two other combined procedures were added to Littler's opponens plasty: widening of the first web space and adductor plasty.
