 Some previous reports have suggested that anterior decompression via the posterior approach for thoracic ossification of the posterior longitudinal ligament (OPLL) results in more frequent intraoperative complications, but that when performed successfully, the clinical outcome is better than after other procedures, such as posterior decompression and fusion. We developed a novel technique that makes it possible to perform anterior decompression for thoracic OPLL safely via the posterior approach. An ultrasonic bone scalpel, the tip of whose hand-piece was curved to reach the OPLL through the side of the spinal cord, was used to perform the novel technique.
